This policy covers the Site only. Information you share with us as a patient in the course of care is handled under separate notices and applicable healthcare privacy laws, which you will receive during intake.

When you submit an appointment request or contact form, we collect the information you enter — typically your name, phone number, email address, and the reason for your inquiry. Contact forms on this Site are processed by Formspree, which delivers your message to us on our behalf.
Please do not include detailed medical history in website forms. Website forms are for scheduling and general inquiries; clinical information is collected through our secure intake process.
Information collected automatically
Like most websites, we and our service providers use cookies and similar technologies that collect certain information automatically, including IP address, browser and device type, pages viewed, and how you arrived at the Site.
We use the Meta (Facebook) Pixel, an advertising measurement tool provided by Meta Platforms, Inc. Text message (SMS) information
If you provide consent during intake, we collect your mobile phone number and a record of your consent so that we can text you about your care. We also retain the content of texts between you and our team.

How we share information
